Fantastic piece of original CO-OP service station ephemera dating to the early 1970s.
This large weekly appointment sheet was designed for use at a CO-OP service station to schedule customers and vehicle repair work from Monday through Saturday. Across the top is the bold green heading “CO-OP SERVICE STATION APPOINTMENTS.”
Each day's section includes spaces for the customer name, time in, estimated shop time, work description and pick-up time. The upper-right corner appears to read “FORM L-500 REV. 7/71,” strongly indicating a July 1971 revision date.
At 22" x 17", this is much larger and more visually interesting than a typical piece of paper advertising. An unusual survivor that would display especially well framed in a garage, workshop, office, man cave or collection of Canadian petroleum/service-station memorabilia.

Condition
Good vintage condition for a paper form more than 50 years old. There are visible wrinkles, creases, handling marks, light staining/soiling and some edge/corner wear. The printing remains clear and the sheet is substantially complete. Please review the photos closely for the exact condition.
I would not flatten, clean or trim it. Its untouched appearance is part of its character.
